2027 PEACE PORTRAITS APPLICATION

If you are nominating someone, please answer the following questions. If you are nominating yourself, you can skip to the next section.

The application fee is $35. Financial assistance is available for individuals who do not have the means to pay at this time. If you are part of a larger organization, we encourage you to request that your organization sponsor your application fee. For more information on financial assistance, please email us at illinoispeaceproject@gmail.com.

If selected, any individual who represents an organization will need to provide permission and references from their organization.

If selected, you hereby grant Illinois Peace Project and its parent organizations permission to use your likeness in a photograph, video or digital or other medium in any and all of its publications, including website entries, or promotional materials without payment or any other consideration. These materials containing your likeness will become the property of Illinois Peace Project and its parent organizations and will not be returned, nor will you be given or have a right to a copy. You hereby irrevocably authorize Illinois Peace Project and its parent organizations to edit, alter, copy, exhibit, publish or distribute your likeness for any lawful purpose.
